After taking impressions of your teeth (both upper and lower arches) custom whitening trays will be made for a one-time fee of $99. You will then be given those custom trays and two (2) tubes of whitening gel. As long as you are current with your regular hygiene appointments (every 6 months including cleaning, x-rays, and exam) you will receive one (1) additional tube of whitening gel at no added cost per year FREE FOR LIFE. Whitening program is void if patient is not compliant with program rules and additional whitening will be at patient's cost. This offer is most suitable for older teenagers and adults and is a $550.00 value. ~SOME EXCLUSIONS APPLY~
